An in home care assessment helps home care companies develop a personalised plan. It is carried out at your home by a care coordinator or care manager and usually lasts around one to two hours. Family members or guardians are invited to take part so that everyone’s routines, preferences and concerns are considered.

What to expect

  1. Initial meeting – A care coordinator or nurse from the home care provider visits the senior’s home along with any family members or guardians. They review daily routines and observe mobility, personal care tasks and social interactions to identify where support may be needed.
  2. Questions about health and lifestyle – Assessors ask detailed questions about medical conditions, medications, mobility aids, diet, sleep patterns, religious practices and favourite activities. Families can share insights about emotional health, mood changes or behavioural concerns, which helps build a well rounded picture of the person’s needs.
  3. Home safety review – The assessor examines lighting, flooring, bathrooms, stairs and emergency exits to spot hazards and recommend modifications to reduce fall risk. Making the home safer supports independence and confidence.
  4. Family involvement – Involving family members ensures that care aligns with the senior’s values and with what relatives can provide. Comprehensive assessments help identify family preferences and abilities, fostering trust and cooperation. Encouraging families to participate in planning and decision making empowers them and ensures the care plan reflects the person’s values and wishes.
  5. Preparing for the visit – Gather medical records, prescriptions, emergency contacts and a written list of routines, challenges and concerns to discuss during the assessment. Clear walkways and good lighting help the assessor observe mobility safely. Encourage your loved one to be present and engaged so they can share their views and build rapport.

After the assessment

The care coordinator uses the information gathered to develop a customised care plan that outlines caregiver duties, visit frequency, safety measures, medication schedules and companionship goals. Families review and approve the plan before services begin, ensuring transparency and trust. The plan is reviewed regularly and updated as needs change, and families receive progress reports and visit summaries to stay informed.
